Program Descriptions

Circle of Help Foundation is an Outpatient Drug and Alcohol Program set up to service a wide variety of clientele. In servicing these clients, Circle of Help Foundation will provide Outpatient counseling services which entails; Group Counseling, Individual Counseling, as well as Treatment Planning and Crisis Intervention services. Following are the descriptions of the modalities which Circle of Help Foundation will be providing services to.


Outpatient Drug-Free

Circle of Help Foundation strives to rehabilitate the client as stated by the guidelines in the Title 22 regulations (which is on file and available upon request) Clients will be serviced until completion of his/her treatment plan, and goals which are determined attainable by the client as well as the counselor in the initial intake assessment. These goals are time sensitive, a six month program completion date is utilized as the standard time frame to rehabilitate the client. If this time frame is not a feasible, the counselor will then refer the client to an additional more intensive service or sign a justification to continue treatment based upon the reasoning in which the client was unable to successfully obtain program completion. During the course of service, a 90 day Treatment Plan update will be administered to assist in redirecting client services as needed.

At any time during treatment the client has either completed a specific goal, which was set in the initial Treatment Plan creation, or has relapsed, in which client has regressed in his/her treatment goals, the counselor will again reassess the client and create a new treatment plan.

Types:
Regular – Is available for both male and female clients who meet the admission criteria.


Day Care Habilitative

Day Care Habilitative services including intake, admission physical examinations, medical direction, treatment planning, individual and group counseling, body specimen screens, medication services, collateral services, and crisis intervention, provided by staff that are lawfully authorized to provide, prescribe and/or order these services within the scope of their practice or licensure.

Day Care Habilitative Services means outpatient counseling and rehabilitation services provided at lease three (3) hours per day, three (3) days per week to persons with substance abuse diagnoses, who are minors, ages 12-21. Clients may live independently, semi-independently or in a supervised residential care facility that does not provide recovery treatment services.

Day Care differs from outpatient drug-free in that day care clients participate according to a minimum attendance schedule and have regularly assigned, structured and supervised activities.

Objectives of day care habilitative services are to:

  • Restore or maintain personal independence in clients with substance abuse impairments;
  • Provide a viable alternative to 24-hour residential care.

Types: Minors Day Care Habilitative Services is available only for medi-cal beneficiaries under the age of 21 whom meet the admission criteria.
